Problem Solving Stipend

Please complete the fields below to receive your $1,000 problem solving stipend. Each organization who participated at the Got Your 6 Summit is eligible to receive one stipend payment regardless of number of participants at the Summit. Organizations must be US based. Organizations must provide proof of 501(c)3 status and a W9. All checks will be made out to the organization and mailed to the organization address entered on this form in July 2022.
